This report is to be completed for any individual involved in an incident while working/volunteering for Rebuilding Together Houston, either in the office or out in the field working on a home. An incident is an event that results in any personal injury, property damage, or production interruption. The facts reported on this form will assist in filing an official report with our insurance should it become necessary.
